There isn't much difference, but the thing to remember is that all Neo games will work on any system, as there are no regional lock outs... however, if you play an English game on a Japanese system, you will get Japanese text, as the language it is displayed in is set by the consoles bios and not the game itself. It works the other way around though - a Japanese game will display English text if it is played on an English system. The only exceptions to this rule are the quiz and Mahjong games, as they are Japanese text only.
You can also get your Neo Geo upgraded with a uni-bios or similar... it is difficult to do on an AES, but if you can find someone who will fit one for you it is more than worth it, because this will let you change language, and also switch between arcade and home console mode, amongst many other things. Something to consider if you buy a Neo Geo for sure!
